A Subject Access Request is a direct factual export from your medical record and doesn't contain medical opinion.  The report can include a list of problems, consultations, medication, immunisations, vaccinations,  letters and results from third parties.  This report is free of charge and can also be adapted to cover a specific time frame.  We would always advise patients to download the NHS APP in the first instance as a majority of this information will be available.
