TRIAGE TELEPHONE SYSTEM
Due to an increase in demand we have now introduced a triage system to improve access to our services. All calls are confidential. The Triage Team are working with and are supervised by the On Day Duty GP. As part of this process all telephone calls will be triaged and you will be asked details of the nature of your call to enable them to assist you more appropriately. The On Day Duty Team consists of an On Day Duty GP and an Advanced Nurse Practictioner(s).
If you are at our Reception and wish to discuss anything in private, please ask a member of the Patient Liasion Team for the use of our dedicated interview room.
You can make an advanced telephone GP consultation appointment by contacting the Practice on 01386 765600 between 8.00am and 6.00pm weekdays or online through Patient Access. When you telephone the Practice to make an appointment with a GP, the Triage Team will ask for a little information to be able to book the next appropriate telephone GP consultation appointment. These GP telephone consultation appointments are available 2 weeks in advance. The Triage Team will no longer arrange direct GP face to face appointments.
If your call is urgent please endeavour to call us between 8.00am and 11.00am (when possible). Your call will be triaged by our Triage Team and if your call is deemed as urgent a message will be given to the On Day Duty Team. They will then assess and make an appointment for you if needed. Please do not request to speak with the On Day Duty Team if your need is not urgent. Our Triage Team can assist with administration requests more efficiently than requesting a call back from the On Day Duty Team.
Urgent appointments are not for repeat prescriptions, fitness for work certificates or for signing forms.
We now offer GP extended hours routine face to face appointments (on occasion telephone consultations) for our patients to be seen (subject to availability) at The Hub which is at The Riverside Surgery, Evesham, Monday to Friday from 6.30pm to 8.00pm and weekends from 8.00am to 12 noon. When the appointment is made for you, you will be asked to consent to registering to this service and for the clinician to view your medical record.
Please speak to the Practice if you wish to be booked into this service.
If you are unable to attend for your appointment, please let us know so that we can offer this to another patient.
The Partners wish to thank you in anticipation of your co-operation with these changes.
