Vale of Evesham PCN- The Wellbeing Team
Social Prescribing aims to support individuals to take greater control of their health and feel more empowered to look after themselves.
Social Prescribers are based within GP practices and secondary care settings. GPs and other health and social care professionals can refer in to the service.
Social Prescribing links patients with non-medical support to address people’s needs in a holistic way. It recognises that people’s health is determined primarily by a range of social, economic and environmental factors and aims to support people to address and deal with issues relating to these factors.
The Vale of Evesham currently has four social prescribers – one specialising in the older adults. We are also in the process of recruiting a social prescriber who will specialise in children/younger adults!

Our trained Lifestyle Advisors will give you the motivational support you need to make small changes to your life, making a big difference to how you feel.
One to one support and group sessions are available for any patient aged 16 and over – all free of charge.
No GP appointment is necessary. You can just refer yourself to the Lifestyle Advisor by filling in an online form or asking our Reception Team to complete the form for you.
Home Oximetry Services: Part of the Covid Management Service
Patients over 65 years of age that are symptomatic with COVID-19, or patients who are clinically extremely vulnerable who are symptomatic with COVID-19, will now be provided with a pulse oximeter to test their oxygen saturation levels for a period of 14 days.
This service is run by the Worcestershire Covid Management Service. Patients should be referred to this service by their GP if they are symptomatic with COVID-19 and require ongoing monitoring at home. (For more information please see the pdf document below)
Home Oximetry Service
We have recently appointed 3 wellbeing coaches to work for the PCN! If you would like to be referred to our wellbeing coaches please speak to your GP.
· Wellbeing Coaches support patients facing low level mental ill health, with one to one coaching sessions or group interventions that enable them to improve their mental wellbeing, coping skills and confidence. Using an individually tailored approach drawing on a range of proven tools. Wellbeing Coaching will build the awareness and self-knowledge that will enable people to take greater control of their own mental health and wellbeing.
