TRIAGE TELEPHONE SYSTEM
Due to an increase in demand we have now introduced a TRIAGE SYSTEM to improve access to our services. All calls are confidential. The TRIAGE TEAM are working with and are supervised by the Duty GP. As part of this process all telephone calls will be triaged and you will be asked details of the nature of your call to enable them to assist you more appropriately.
The ON DAY DUTY TEAM consists of a Duty GP and a Minor Aliment Clinic.
If you are at our Reception and wish to discuss anything in private, please ask a member of the Receptionist Team for the use of our dedicated interview room.
All surgeries are by appointment and can be made in person, by telephone and on-line. You are able to access appointments online for your Responsible Doctor, Salaried and Locum Doctor. Practice Nurse, Health Care Assistant and routine Doctor appointments are available to pre-book up to 4 weeks in advance.
GP’s will no longer do routine telephone consultations on the day you telephone. You are now able to pre-book a routine telephone consultation.
You will no longer be asked to telephone us back on another day for the opening of a routine appointment. We will take the details and try to accommodate you there and then or we will telephone you back with an appropriate appointment.
If you feel your need is ‘Urgent and Necessary’ for the day, please contact the Surgery between 8.00-11.00am. Our Triage Team (under supervision of a GP) will ask for clinical information in order to direct you to the most appropriate clinician. If your clinical need is appropriate for the Minor Ailment Clinic they will book an appointment with a Nurse Practitioner or alternatively arrange for you to have a telephone consultation from a clinician and our ON DAY DUTY TEAM will assess your need over the telephone. The ON DAY DUTY TEAM will book a face to face appointment for you if necessary.
Urgent appointments are not for repeat prescriptions, fitness for work certificates or for signing forms.
We now offer GP extended hours routine face to face appointments (on occasion telephone consultations) for our patients to be seen (subject to availability) at The Hub which is at the Riverside Surgery, Evesham, Monday to Friday from 6.30-8.00pm and weekends from 8.00am-12noon. When the appointment is made for you, you will be asked to consent to registering to this service and for the clinician to view your medical record.
Please speak to the Surgery if you wish to be booked into this service.
If you are unable to attend for your appointment, please let us know so that we can offer this to another patient.
The Partners wish to thank you in anticipation of your co-operation with these changes.
